Waikato
Women’s Hospital
I dropped my commitment to
Waikato hospital in 2004 to further my goal of setting up a private practice.
However, I have continued my clinical commitments to the areas of my interest
and expertise at Waikato Hospital. I
also have a successful Gynaecology and Reproductive Medicine practice based at
Anglesea Women’s Specialists and Fertility Associates Hamilton respectively. I
helped create a Fellowship in endoscopy and urogynaecology in 2005 aimed at
training a senior registrar in the last year of their training. This position
is a joint appointment between Waikato DHB and private sector and forms a model
for future surgical training in O&G. I was the convener of the RANZCOG NZ Committee ASM in April 08.
I have a keen interest in
pelvic floor surgery and have proposed a comprehensive pelvic floor clinic
based at the department of O&G that will have input form urologists and
colorectal colleagues. I have been at the forefront of developments in mesh
surgery for pelvic floor repair as well as urinary incontinence and have
performed a large number of vaginal mesh repairs with good efficacy and safety.
I have also conducted many workshops in pelvic floor surgery for colleagues in
the department as well those for other DHBs.
I am passionate about
endoscopic surgery and have championed the skill development in this area over
the last 8 years. I continue to develop the endometriosis service and am
involved with training other colleagues as well as the registrars in modern
excisional techniques.
I was involved with the
setting up of Clinical Risk Management Team and chaired this committee for some
time. I continue to assist the
departmental audit programme through training and guidance of RMOs and Trainee
Interns. I have a keen research focus and have assisted the registrars in their
research projects.
I have made significant
contributions to Surgical Skills development by conductive dry and wet animal
lab workshops. I believe my
contributions have assisted in building our reputation as a centre for
excellence in the areas of endometriosis and pelvic floor surgery as evidenced
by Royal College Report 2005. I have had requests from many colleagues in other
DHBs for preceptorship in the area of pelvic floor and endometriosis.
Associate Medical Director since June 2005
Key Responsibilities in this role are-
Since accepting this role
in June 2005 I have brought in a number of changes in the above areas that have
helped the success and growth of the clinic. FAH is an extremely well run,
efficient, high quality and friendly organisation.
I was involved with setting
up of the first group practice in O&G in Waikato with the goal of providing
a comprehensive, peer reviewed and high quality service to women of midland
region. I have an extensive second opinion practice in Endometriosis, Pelvic
Floor Surgery and Reproductive Medicine. I enjoy operating privileges at
Southern Cross and Braemar Hospitals and conduct approximately 2 operating
sessions a week. This period has seen a 100 percent growth in the overall
business of the group.
April 2001-June 2004
In this
period I continued to develop my advanced laparposcopic skills particularly in
area of complex endometriotic surgery. I pioneered the excisional surgery for
endometriosis at Waikato and performed a large of these procedures with high
level of safety and efficacy. I also actively encouraged teaching and training
in endometriosis.
My
other major interest was the development of a pelvic floor service. I was
instrumental in introducing the modern methods of mesh repair for stress
incontinence as well as prolapse at Waikato Hospital. These have been shown to
have much lower failure rate and complications rates have been very low. I
organised a number of training workshops for in house training as well as
training of colleagues outside the DHB.
My work
in the area of endometriosis and pelvic floor surgery in this period was widely
appreciated by colleagues in the department as well as those from other centres
in New Zealand.
I was
actively involved in performing colposcopies at Waikato Women’s. I continued my
involvement with obstetrics by conducting one antenatal clinic a week. I shared
the on call work load with my other colleagues. I was appointed a member of the
executive committee of the Perinatal Society of New Zealand. I assisted in the
formation of Clinical Risk Management Team (CRMT) and was nominated the SMO
representative on this committee.
I
actively participated in the teaching and training of junior medical staff,
nurses and midwives. I developed a CTG training programme for junior medical
staff and midwives in this period. I conducted regular CTG teaching sessions
using this package.
Surgical
skills’ training was a particular area of interest for me. I was invited to be
a tutor for the basic surgical skills training at Greenlane /National Women’s
Hospital and thoroughly enjoyed being a part of the faculty. I was involved
with setting up a surgical skills centre at Waikato Hospital and have conducted
a number of skills workshop on advanced laparoscopic skills at Waikato
Hospital.
My
interest in the area of assisted reproduction saw me work at Fertility
Associates Hamilton/ Isis clinic for two sessions a week. The clinic has gone
from strength to strength since its inception and I am proud to have
contributed to its success.

Appointed
as full time Registrar (Training position) in the Department of
Obstetrics and Gynaecology.
I
was fortunate to be part of an extremely friendly and helpful team. During this
period I furthered my experience in areas of Gynaecological Oncology, Ultrasonography
and Endoscopic surgery besides the routine O & G workload.
The
Royal New Zealand College of Obstetricians and Gynaecologists on behalf of the
Medical Council of New Zealand interviewed me in May 1998. I was asked to take
the MRANZCOG written and oral exams and complete 2 years of logged work as a
part of my assessment for suitability for Vocational Registration in O&G in
New Zealand. I passed my MRANZCOG (written) examination in August 1998 at first
attempt and completed two years of logged training. During this period I passed
the MRANZCOG (oral) examination at first attempt and completed the In Hospital
Assessments Modules in the areas of consultation skills, ultrasonography and
colposcopy.
I
was granted the Fellowship of Royal Australia and New Zealand College of
Obstetricians and Gynaecologists in August 2000. I was subsequently included on
the Vocational Register in the branch of Obstetrics and Gynaecology by the
Medical Council of New Zealand.
I
actively participated in the training and teaching of Trainee Interns and
Senior House Officers. I also had a significant role in organising and
imparting training to the Junior Registrars during this period.
I
presented a number of papers at grand rounds and international congresses
during my tenure as registrar at Waikato Hospital. I was invited as a guest
speaker at the 43rd All India Congress of O & G in December 1999
and contributed to a colposcopy workshop and the scientific session on “New
Frontiers in Gynaecological Oncology”.
I
was actively involved with research activities at Waikato Women’s and completed
a prospective study, which has been sent for publication. I undertook and
instigated a number of audits and coordinated an annual report for the
department. My keen interest in computers saw me provide support to senior and
junior medical staff on many occasions.

Worked
as Registrar in Obstetrics & Gynaecology at KEM Hospital, Bombay,
under Professor Shirish S Seth. He is one of the leading teachers of
gynaecological surgery in the country and has won worldwide acclaim for his
work in vaginal hysterectomy. He was recently appointed the President of FIGO.
With Professor Seth there were opportunities for first hand experience of
diagnostic and operative endoscopy, difficult vaginal hysterectomies and pelvic
floor repairs, various surgeries for stress incontinence and vaginoplasty. I
was fortunate to be actively involved in organising a 2 days workshop on “
Operative management of Stress Urinary Incontinence” by Mr Stuart Stanton.
Research
activity during this period included a trial of conservative management of
Intrauterine Foetal Death cases.
August
1989 - January 1991

Bombay
Worked as Registrar in Obstetrics & Gynaecology at KEM Hospital under Professor I N Hinduja.
Prof
Hinduja is credited with bringing the technology of IVF to India and the very
first IVF baby of country was born at KEM Hospital in1987. I was fascinated by
the advancements in the area of infertility and considered myself fortunate to
be working with such a prestigious team.
Under
her able guidance, I learned the various techniques of ovulation induction
including GnRH analogues, reproductive endocrinology, transvaginal sonography,
intrauterine insemination with washed semen, gamete intra-fallopian transfer,
in vitro fertilisation embryo transfer (IVF-ET), egg donation and surrogacy,
prenatal diagnostic techniques, etc. I was responsible for conducting a trial
on intrauterine insemination with washed sperms in hyper stimulated cycles. I
contributed to various research
projects in the area of infertility. My paper in the annual Bombay O & G
Congress was judged the best paper in the subject of assisted reproduction.
I
had the additional responsibility of teaching the undergraduate medical
students and student nurses in the form of theory and practical classes. I was
expected to handle routine obstetric emergencies independently and had the
opportunity to perform various gynaecological surgeries, both major and minor,
in this period. My ambitions of progressing in the area of IVF and assisted
reproduction were not fully realised, as the publicly funded IVF programme at
the KEM Hospital had to be terminated in 1991 due to the lack of government
funding.
February
1988 - July 1989

the guidance of senior colleagues. I also learnt to perform various elective
& emergency procedures like D&C, Hysteroscopy, Medical T.O.P,
instrumental deliveries, caesarean section, and tubal ligation under
supervision. I also gained valuable experience in the area of the infertility
viz history taking, initial infertility work up & counselling, AIH and AID.
I also had the unique opportunity of assisting at and observing the various
assisted reproduction techniques of IVF-ET, GIFT, IUI, etc during this time.
